You can schedule your physical therapy appointment using the link to the right, where you’ll find all visit types, descriptions, and pricing. If this is your first visit, please book an Initial Evaluation. For follow-up appointments, select the duration discussed during your initial visit.

We have a wide array of coaching plans available to fit your needs and level of financial commitment! Simply fill out the Nutrition Coaching Application at the button to the right, and a Momentum Coach will reach out to you to get started, discussing the details and options of each individual plan.

Plans range from $175.00 to $1300.00
